ideally U4's and L:5's but....good 18 months trt

compromise...lower incisor and upper laterals out and composite on the canines.....
or
lower incisor and upper canines out and bring laterals forward


upper is gonna take some time - so if you went for u4s I would leave the lwoer for a good 9 months....then start the lwoer so they both finish togther if you ge try drift

Hi Ian,

Can you take a look at this case.
Skeletal slightly 3
Slightly Reduced LAFH
Narrow upper lip and slightly acute NLA

Upper severe crowding, laterals in massive crossbite and UR5 is 90degree rotated.
Lowers mod-severe crowding.
Molars  class 1
canines class 2 1/2 unit
incisors class 3 and retroclined.

Obvious patient complaint is laterals 'shark teeth' like a double row of teeth and canines pointy and yellow.

Complicated one but my thoughts: if the molars are in a reasonable place then would you suggest upper 4s and lower 5s in this case? Obviously the laterals will be a bit of work to get in line... open coil between 1-3 and closing springs 3-6, then get an eyelet bracket on the 2s when possible. Inverted lateral brackets to give some buccal root torque?
Or would you extract upper 5s to get rid of the rotated UR5 and make the posteriors easier?
Do you think it's a workable case?
