I d go for for both upper 4's as both molars are a full unit class 2... and class 2 elastics

Pt does not like the way her teeth are wonky.

The patient is a mild class 2 skeletal pattern, with average FMPA and average FH. The lips are competent and full with a 90 degree nasiolabial angle.

The incisors are a class 1 with an OJ of 2-3mm and the OB is increased by 75%. centre line on the lower is OK but upper its to the right by 2mm

There is moderate crowding on the upper labial segment and mild crowding in the lower labial segment. The upper and lower buccal segments are aligned. The UR3 are displaced around 5mm buccally.

Molars on the LHS are class 2 and the RHS are class 2. The canines on the LHS and RHS are class 2 half a unit.

this has to be an extraction case due to the crowding the upper arch - was thinking UR4 only but then the centre line will be off? then upper and lower fixed and class 2 elastics????
